Hospice and Palliative care Utilization and Associated Factor of Terminal Cancer Patients in Korea

Abstract
This study aims to identify Hospice and Palliative care(HPC) utilization and associated factors of terminal cancer patients in Korea, and to further provide an empirical evidence of HPC expansion policy. To evaluate HPC by terminal cancer patients in the last year of life, data were used 2019 deaths from the National Health Insurance Database. Among all terminal cancer patients, 22.1% received a HPC, and associated with gender, age, SES, geographic residence, and type of cancer. Findings revealed that male, individuals with low socio-economic status, residing in rural areas, and ¡®malignant neoplasms of lymphoid, hematopoietic and related tissue¡¯, ¡®neoplasms of uncertain or unknown behavior¡¯ were less likely to use HPC. HPC patients were significantly related to lower rate of aggressive care. By the provision type, the HPC is classified into hospitalization, consultation, and home-based treatment. Among all HPC patients, 67.0% received a hospitalization only, 11.2% received a hospitalization and consultation, 11.1% received a consultation only, in order. HPC is needed to ensure dignity and quality of life in all individuals, regardless different circumstances. This study provides a reference for decision-making in the future expand of HPC.
